**Approach:** Over 500 references were collected and evaluated. This study presents the major steps in the evolution of surgical microscopy. An in-depth technical overview of optical systems, mechanical systems, lighting, visualization, and integration with advanced imaging technologies is provided. Various medical applications of the surgical microscope are discussed in [neurosurgery](https://www.mindstick.com/blog/300917/brains-work-via-their-genes-just-as-much-as-their-neurons) and spine surgery, ophthalmic surgery, otolaryngology (ENT) surgery, root canal therapy, and plastic and reconstructive surgery.

**Results:** Surgical microscopy has advanced significantly in terms of high-performance optics, bright and shadow-free illumination, reliable and flexible mechanical design, and comprehensive visualization. New imaging technologies such as hyperspectral imaging, OCT, fluorescence imaging, optoacoustic microscopy, and laser spot contrast imaging are integrated with surgical microscopes. Advanced visualization and AR are being added to surgical microscopes as new capabilities that will transform clinical practice in the operating room.

**Conclusion:** The combination of new imaging technologies and surgical microscopes enables surgeons to perform complex procedures and improve surgical outcomes. Improved visualization and improved ergonomics have made surgical microscopes a powerful tool in neurosurgery, spine, otolaryngology, ophthalmology, and plastic and reconstructive surgery.
